Nereus (tradipitant)

• For diagnosis of Prevention of Vomiting
  Indicated for the prevention of vomiting induced by motion in adults

Criteria

Prior Authorization

Length of Approval: 1 Month(s)

• Used for the short-term prevention of vomiting induced by motion
AND
• Patient is 18 years of age or older
AND
• Dosing frequency will be limited to once daily dosing
AND
• Trial and failure, contraindication, or intolerance to two conventional motion sickness therapies including but not limited to the following: environmental modification, transdermal scopolamine and antihistamines (e.g., dimenhydrinate, meclizine) [3] [4]
AND
• Requested drug is not being prescribed for off-label use (e.g., drug induced nausea and vomiting, morning sickness, vertigo)
