Incentives Official Rules

Eligibility

This incentive program is open to WHA members who:

• Are California residents and currently live or work in one of the following counties: Alameda, Colusa, Contra Costa, El Dorado, Marin, Napa, Placer, Sacramento, San Francisco, Solano, Sonoma, or Yolo.

• Have successfully completed a specified preventive exam (mammogram, pap, dilated retinal test, A1C or other specified preventive test noted in WHA incentive) within the past year.

• Are currently an active member, commercial and individual members only.  Medicare members are not eligible.

Note: Employees of WHA and their immediate family members are not eligible to participate.

General rules

All federal, state, and local laws and regulations apply. Void where prohibited by law. All taxes, fees, and surcharges on the prize are the sole responsibility of the winner. The winner will be notified by email or phone call. If the winner cannot be reached at the email address or phone number submitted with entry, the prize will be forfeited and an alternate winner will be selected. By participating in this incentive program, participants agree to be bound by these Official Rules and the decisions of WHA regarding administration of the incentive program are binding and final. Failure to comply with these Official Rules will result in disqualification.

How to enter 

To enter, you must complete the entry form located on WHA's website. Incomplete submissions may not be eligible for the drawing.

Odds of winning

Qualifying participants are eligible to have their name drawn monthly for a period of 12 months from the date of the exam. The odds of winning are approximately 1 in 150. 

Selection of winners 

The selection of winners will be conducted by a random monthly drawing during the first week of each month from all eligible entries received within the applicable 12-month period. 

Release of liability 

Participants agree to hold harmless WHA and its employees from any and all liability, claims, demands, or causes of action for personal injury and damages arising from participation in the sweepstakes and use of the prize.

Use of Name, Photograph, and Likeness

Acceptance of the prize constitutes permission for WHA to use the winner's name, photograph, and likeness for purposes of advertising, trade, publicity, and promotional purposes without further compensation, in any and all media, without notice, review, or approval.
