Available for employer group members that have/been diagnosed with hypertension, Livongo for Hypertension incorporates a suite of tools including a connected blood pressure monitor, real-time insights after each reading, health education, and one-on-one support from expert coaches. This individualized care and equipment is included with your WHA health plan*, so there is no added cost to members.
Members simply enroll in the program, and Livongo will mail you a Welcome Kit and an advanced blood pressure monitor. Then, members may access Livongo’ s wireless, web-based hypertension portal to get vital information about your readings, managing your condition with lifestyle changes, and receive helpful information from coaches who review your results and provide personalized feedback.
