WHAT IS THE REQUIRED CONTRIBUTION?
Employers must contribute a minimum of 50% of their employee's premium for the base plan (or lowest cost plan).
Dependent contribution is at the employer's discretion.
HOW MANY PLANS CAN I CHOOSE?
Your quote should reflect your base plan. During the application process, you can select up to three plan options,
as long as you have a minimum of three enrolled employees.
Please note: Only one HSA-compatible high-deductible health plan can be included in your plan options.
