SELF ADMINISTRATION - Oral

Indications for Prior Authorization:

• Treatment of moderate to severe vasomotor symptoms due to menopause

Patients must meet the following criteria for the indication(s) above:

• Patient has a diagnosis of vasomotor symptoms associated with menopause, AND
• Patient has completed a trial of and had an inadequate response to or intolerance to oral estradiol 1 mg tablets and progesterone 100 mg capsules used concomitantly as separate products as documented in chart notes

Dosing:

• One 1 mg/100 mg capsule orally each evening with food

Approval: 

• 1 year
